| Information in your PowerPoint entry must focus on Dental Health! |
1.) Do you have your Entry Form- .PDF File completed, including the information and your parent or guardian's signature at the bottom of the form?
2.) Did you save the completed Entry Form-Rubik as a (.pdf, .doc, or other type file) which can be attached to email?
3.) Is your PowerPoint Project or the like, saved as a file which can be attached to email?
4.) Make sure you also include who you are in the body of the email...Your Name, Address and Phone Number.
5.) If you have difficulty sending both attachments in one email you may send them individually. Make sure you put your Name, Address and Phone Number in the body of both emails.
6.) When you are ready; a.) CLICK on the following LINK. b.) Fill in your Name, Address and Phone Number. c.) Attach your completed Entry Form- and completed PowerPoint Project file!
