1. Create a Fun, Persuasive Power Point Presentation. CLICK HERE for STUDENT PROJECTS+CONTEST
2. To be eligible to enter, you must be a student in 6th grade to 12th grade with your permanent residence in the zip code area identified below.
3. Topics: "Turn Up Your Smile Power!" or "Power Up Your Smile!"
4. Presentation should relate to or encourage excellent dental health.
5. You are welcomed and encouraged to design your presentation with animation and sound. Think "out of the box" and have fun!
6. This Contest is limited to students in the zip code areas provided. (Contact us for exceptions if you believe your zip code should be on this list due being within the same geographical area) CLICK HERE FOR ZIP CODE LIST 7. Entry Deadline is January 15, 2009.
8. Submit your Entry Form & PowerPoint by Email or burn a CD and Mail. 9. Winners will be announced on this website and Prizes awarded in during National Dental Health Month - February 2009. Winners will be notified through US Mail. Winners may also be contacted by email or telephone.
10. The entry form and all information thereon and all entries, including all PowerPoint's and CDs and the entire content thereof, upon receipt by Lee F. Peterson, DDS, shall automatically become the sole property of Lee F. Peterson, DDS and Lee F. Peterson DDS and its affiliates are hereby granted the right to use the same in websites, printed materials, advertising, promotions, publications and for any other lawful purpose.
